Management reserves the right to refuse any Exhibitor. Upon refusal, Management will refund all monies that accompany these forms. All booths will be reserved on a first come, first served basis. 50% non-refundable deposit is due with application submittal and is needed to hold booth space for the exhibitor. Balance of booth fee is due by August 31, 2011. No booth will be given without full payment. All applications must have at least the deposit to be considered for space at Inktoberfest 2011. If you fail to submit ALL forms and payments in full to Aaron Antonucci, Adrenaline Tattoo by said date, your spot will be sold and your deposit will be forfeited. Please be sure to fill out the application for the Mississippi State Health Department in full, attach copy of current Blood borne Pathogen Certificate, passport photo, and copy of DL, ID or Social Security Card. An Application for each artist attending is required. The cost of the booth includes two (2). A double booth (10x20) includes four artists. Do not send directly to the Department of Health the health department will reject without full payment and for being partially filled out. All applications must be approved by Management prior to Health Department Submittal. |
